- Other services may be subject to copayments, coinsurance or deductibles
- Out-of-network inpatient stays exceeding 48 hours (96 hours for C-section) require prenotification or benefits may be reduced to 50% of allowed amount

Infertility Benefits
Blue Cross Blue Shield coverage for employees and spouses includes:
- Diagnostic testing and evaluation
- Treatment of underlying medical conditions
- Assisted reproductive technologies (ART), including:
- IVF (in vitro fertilization)
- IUI (intrauterine insemination)
- GIFT and ZIFT
- Artificial insemination
Eligibility Requirements
Individuals under age 35 may qualify for infertility benefits if they are unable to become pregnant after one year of regular, unprotected intercourse or therapeutic donor insemination.

The Spring Health Employee Assistance Program (EAP) offers confidential support for reproductive health–related topics, including:

Postpartum support
Employees and household members receive up to 10 free counseling sessions per year. Additional care can be coordinated through your health plan to help reduce out-of-pocket costs.
